Driver Document List
Employment Application
Following information is necessary to process the driver’s application: the driver’s full name, date, and signature. Additionally, we need the name and address of the employing motor carrier. If the driver’s religious practices prevent them from providing their Social Security Number (SSN), including it is unnecessary.
Please provide a complete three-year residence history without any gaps. For the employment history, you must be having proof of three years of previous employment. If the driver has worked in a safety-sensitive position, there may be the need for up to an additional seven years of employment history (a total of ten years). Each employer should be listed with their company name, address, supervisor’s name, contact information, and reason for leaving. If there are any gaps in the employment history exceeding 30 days, please explain.
CDL
To ensure compliance, it is crucial to have the following documents on file for drivers:
- Valid Commercial Driver’s License (CDL): Please provide clear scans of both sides of your CDL. If your state includes endorsements or restrictions on the back of the license, please include those as well.
- Single CDL Limitation: Drivers can possess only one valid CDL unless they have received a special exemption from the Department of Transportation (DOT). If such an exemption has been granted, please keep a copy in the Driver Qualification (DQ) folder.
- Original CDL Retention: It is recommended to retain the original CDL of a driver in the DQ folder throughout their employment with our company. This proves that we have done the necessary due diligence during onboarding.
- Non-CDL Drivers: Non-CDL drivers must also provide a valid state driver’s license that is not expired. If applicable, ensure the license includes the appropriate endorsements.

Renewing your HUT permit can be done conveniently either through the official website of the New York State Department of Taxation and Finance or by utilizing the services of an approved permit service company. The renewal process typically takes a few weeks to finalize, ensuring compliance with the requirements.
MVR
The Motor Vehicle Report pertains to the immediately preceding three-year timeframe (in each state where the driver possessed a valid license).
The MVR must be requested within 30 days from the driver’s official employment date. A fresh MVR must be conducted before granting driving privileges if the driver is not hired within this timeframe.
